- What is the Advanced Primary Care Management (APCM) Program?
Our program is designed to offer enhanced support and care coordination tailored specifically to your health needs. Through this program, you will have access to:
- A Dedicated Care Team: Including your primary care provider, care coordinators, and other healthcare professionals.
- Personalized Care Plans: Customized care plans focusing on managing chronic conditions, preventive care, and achieving your personal health goals.
- Proactive Health Monitoring: Regular check-ins and health monitoring to ensure your care stays on track.
- Convenient Access: Same-day or next-day appointments, telehealth visits, and extended office hours.
- Important Program Information
- This service is covered by Medicare.
- Only one provider can furnish and be paid for APCM services during a calendar month.
- You have the right to stop services at any time.
- Cost sharing may apply to you based on your Medicare coverage, but it will typically be covered by your secondary insurance, if applicable.
