Our program is designed to offer enhanced support and care coordination tailored specifically to your health needs. Through this program, you will have access to:

  • A Dedicated Care Team: Including your primary care provider, care coordinators, and other healthcare professionals. 
  • Personalized Care Plans: Customized care plans focusing on managing chronic conditions, preventive care, and achieving your personal health goals.
  • Proactive Health Monitoring: Regular check-ins and health monitoring to ensure your care stays on track.
  • Convenient Access: Same-day or next-day appointments, telehealth visits, and extended office hours.

  • This service is covered by Medicare.
  • Only one provider can furnish and be paid for APCM services during a calendar month.
  • You have the right to stop services at any time.
  • Cost sharing may apply to you based on your Medicare coverage, but it will typically be covered by your secondary insurance, if applicable.
