We know you have many questions about access to COVID-19 vaccinations. We are writing to share information with you, including the latest on vaccine supply and updates to eligibility.
Where Can I Get the Vaccine?
Unfortunately our office does not offer the COVID-19 vaccination at this time. If that changes, please know that we will reach out as soon as possible.
To see if you are eligible to receive the COVID-19 vaccination, follow the directions on the site based on where you live:
At this point in time, most states are vaccinating 1A and 1B groups. 1B includes:
Individuals at High Risk (Phase 1B)
Individuals aged 65 and older, and individuals ages 16-64 with medical conditions, as defined by the Centers for Disease Control and Prevention, that increase the risk of severe illness from the virus. These conditions include:
Individuals who are pregnant and those in an immunocompromised state (weakened immune system) from solid organ transplant are also eligible but should follow CDC guidance and first discuss vaccination with their medical provider before receiving the vaccine.
What should I do if I’m Not Sure I’m Eligible?
Please locate and complete the pre-screening eligibility form for your city or state listed above under ‘Where can I get the Vaccine?’.
This will determine if you are currently eligible.
How Do I Prove That I Am Eligible?
To receive vaccination, you will need to provide proof of eligibility. For those who are eligible due to a medical condition, you will need a letter from your doctor, medical information that documents your medical condition, or a signed certification.
